About the Diabetes Control Center

OFFICE HOURS. Mon-Thurs 8am -1pm and 2pm-5pm. Closed for lunch from 1pm -2pm.

The Diabetes Control Center is focused on the treatment of diabetes. It is necessary that all our patients have a PRIMARY CARE OR FAMILY PHYSICIAN. While we may occasionally have treatment suggestions in areas other than diabetes, we will not treat other problems such as colds, flu, arthritis, etc.

We do not admit to or consult on patients in the hospital. A progress note will be sent to your PRIMARY CARE OR FAMILY PHYSICIAN after each visit to the Diabetes Control Center.

APPOINTMENTS. Appointments are made as close to your requests as our schedule permits. We will attempt to remind you of an appointment with an automated call to the phone number you provide. It is your responsibility to remember the time/date of and keep your appointments.

MISSED APPOINTMENTS. Missed appoints are costly. A charge will occur if you miss an appointment or fail to cancel 24 hours before the scheduled time.

WHAT TO BRING. Picture ID. All insurance and drug cards. Your glucose meter (at every visit). Insurance Co-pay. All medicines currently taking. Any Medical records you have.

WHAT NOT TO BRING. Do not bring small children to the Center. They cannot be left unattended in waiting areas.

WHAT TO WEAR. Clothing that is easily removed. A jump suit is ideal. Shoes and socks that are easily removed. Ladies, please to not wear panty hose.

To save time you may complete the PATIENT INFORMATION FORM online and print it. You will not be able to save the completed form on your computer. You could then either mail, fax (803-536-6734) or bring it with you.
