What you need

  1. A glucose meter; or other device with the correct time and date.
  2. An account with TIDEPOOL

  3. A laptop or desktop computer with Google Chrome
  4. A USB cord.

What you will need to do

  1. Download and install the Tidepool uploader

If you have a Medtronic pump and a PC, MacBook, laptop or Chromebook.

You will need a CareLink USB –shipped when you got your pump.  A video is here: https://youtu.be/u_oC2EubVT4

  1. If you have not done so, create a CareLink account Medtronic. https://carelink.minimed.com/patient/entry.jsp?bhcp=1
  2. Log in.
  3. Select what you are to upload: MiniMed insulin pump; Gradian Real Time or blood glucose meter. Generally, you will select pump. If using IOS have a problem, a trouble shooting video is here: https://youtu.be/iP1L4Xskl7I
  4. Plug the CareLink USB in the USB port of your computer and follow the screen prompts.
  5. To allow Dr. Raine to see the reports please send an email to info@diabetesSC.com with your CareLink username and password, informing me of your upload. I will contact you to arrange a telemedicine visit.
